Live-out Services
New clients who contract for live-out services through CareTrak Home Care and who use a minimum
of twelve (12) hours of CareTrak’s live-out services per week for the first 4 consecutive weeks of services will be eligible to receive a $75 service credit at the end of the 4th
week.
Live-in Services
New clients who contract for live-in services through CareTrak Home Care and who use seven (7) live-in days of CareTrak’s services per week for the first 4 consecutive weeks of services will be eligible to receive a service credit equal to two (2) live-in days of caregiver services. This credit will be applied at the end of the 4th week. If the client maintains seven (7) live-in days of CareTrak’s services per week for the following 4 consecutive weeks the client will be eligible to receive an additional service credit equal to one (1) live-in day of caregiver services. The additional credit will be applied at the end of the 8th week.
New clients who contract for live-in services through CareTrak Home Care and use four (4) to six (6) live-in days of CareTrak’s services per week for the first 4 consecutive weeks of services will be eligible
to receive a service credit equal to one (1) live-in day of caregiver services. This credit will be applied at the end of the 4th week. No additional credits will be applied.
Service credits will be issued after all eligibility requirements have been met. Once applied, there will be no additional service credits for that client under this program. This is a one time only, limited offer. Special terms and conditions apply.
