
Only first time clients are eligible for this one time, limited offer. The prospective client must mention the 96.9 WTKK radio ad when they initially contact CareTrak Home Care. In order to be eligible for the service credits, the new client must fulfill the terms of the CareTrak Service Agreement throughout the eligibility period. Before CareTrak issues a service credit, the outstanding account balance must be paid current and any payments must be processed. Service credits are only applied to future CareTrak caregiver services and are not redeemable for a cash value. Service credits may not be used to satisfy adjustments to the client’s security deposit or to reduce transportation, travel service, late fees or other account balances. Service credits are not transferable to any other person or business entity. CareTrak reserves the rights to change or cancel this special offer at any time, and for any reason without notice.
CareTrak will confirm a new client’s eligibility to participate in the service credit program at the time the Service Agreement is signed. At the end of the eligibility period, CareTrak will determine if the new client satisfied all of the requirements for the service credit. If all requirements were met, CareTrak will automatically apply the service credit to the clients account. The credit will appear on the client’s statement as an adjustment for future services.
